Can I export followers?

Can I send DMs?

Yes, from accounts you connect, once account connections ship. Never from shared accounts.

Is it real time?

Reads are fetched live when you call. Streaming webhooks aren't available yet.

Do you show whether a user accepts DMs?

Yes. User objects include canDm when X reports it. Otherwise it's null.
